National Diploma (ND) Admission Requirements

- Candidates must have participated in the 2026 UTME.
- Required UTME subjects include:
English Language
Biology
Physics
Chemistry
- Minimum score will be determined by the school.
- Candidates must possess at least five (5) O’Level credit passes in:
English Language
Mathematics
Physics
Chemistry
Biology
- Results must be obtained at not more than two sittings.
- NABTEB is accepted but cannot be combined with another O’Level result.
- Candidates must not be less than 16 years old at the time of application.
- Candidates must not have criminal records.
- Candidates must not have hand deformities.

Higher National Diploma (HND) Admission Requirements

- Candidates must meet the requirements for National Diploma admission.
- Candidates must possess a National Diploma certificate or Health Record Technician certificate.
- Candidates with ND in Health Information Management must present evidence of one year Industrial Training.
- Candidates must possess Professional License Certificate (Part I) issued by the Health Records Officers Registration Board of Nigeria (HRORBN).

Closing Date

- Completed application forms must be submitted on or before 24th August, 2026.

Screening and Interview

Written examination and oral interview dates will be communicated later.

Other Relevant Information
- Late submission of application forms will not be entertained.
- Candidates with unclear passport photographs will not be admitted for screening.
- Candidates who fail to appear for screening as scheduled will not be considered for admission.
- Applicants must come with their application slip and writing materials.
- GSM phones and electronic devices are prohibited during the screening exercise.
- All photocopies of documents must be clear and legible.
- Evidence of change of name where applicable must be attached.
- Screening results will be published on the UITH website and notice boards.

Application Process After Payment
- Proceed with online registration using the generated Remita Retrieval Reference (RRR) Number.
- Enter a valid personal email address.
- A confirmation link will be sent to the registered email address.
- Open the link sent from noreply@uithilorin.
- Complete and submit the online SHIM application form.
- Upload all required documents correctly.
